MOTORCYCLE TOWING / BIKE ASSIST : MOTO AID MALAYSIA: MEMBERSHIP REGISTRATION FORMS



Dear Bikers,

To ease your registration with Motoaid Malaysia via email / phone , do download our official form here.

Steps:

1. Download and print out the form below
2. Fill in the necessary. Compulsory are :a)  name
                                                                  b)  ic number
                                                                  c) contact number
                                                                  d) valid current postal address
                                                                  c) motorcycle's number plate
                                                                  d) motorcycle's model
                                                                  e) motorcycle's capacity (cc)
                                                                  f) current motorcycle's color
                                                                  g) emergency contact name
                                                                  h) emergency contact number
                                                                  i) relationship with the emergency contact person
                                                               
                                                                   Optional:
                                                                   a) blood group / type
                                                                   b) etc.
3. Send back the completed forms to either:
                                                                   a) motoaidservices@gmail.com (attention to cards dept)
                                                                   b) whats-app to : 014-2458878 

 4. Do inform us by sms to 014-2458878 / 016-3778183 or 011-26407873 that the form has been forwarded to us

5. Our officers will get back to you asap after the inspection of the forms to see if the information provided is sufficient

6. Make your deposits (with the corresponding amount) to our account :
 MOTO AID SERVICES (M) SDN. BHD.
CIMB : 1265 000 172 8052 

7. Kindly inform forward us of your payment and do send us the copy/picture of  payment slip via email or by whats-app.

8. Your registration will be activated on the spot via the receipt of an official sms by Motoaid to your mobile phone number provided for prior, in the registration form.

9. Successful registration will be followed by the posting of a special Motoaid Membership Card , official stickers and supporting documentation in not more than 4 working weeks.
